Patient Requirements

In order to be considered for weight loss surgery we require the following from you:

1. Attendance of the information session (education class)
2. Recommended attendance of support groups prior to surgery.
3. Initial Consultation with Bariatric Nurse and Surgeon

Please bring the following to your initial visit:

• Your insurance card
• You must ensure that your health insurance coverage includes Weight Loss/Bariatric surgical benefits
• Completed registration/questionnaire package link to document on site
• Insurance referral if your insurance company requires one

At the end of your consultation, you will be scheduled for some of the following appointments:

• An appointment with our nutritionist
• An appointment with a psychologist
• Comprehensive blood testing
• Other specialty evaluation, if needed.

Surgery Scheduling

Depending on your insurance company, the time interval until surgery will vary.  Some insurers now require 3-6 months of medically supervised weight loss, which we will assist you with.  We strive to make the process as efficient and seamless as possible for you.
